Your appointment time has been reserved just for you. We understand that cancellations are sometimes necessary. However, we request 24-hour
notice if you need to change your scheduled time. We also ask that you arrive to your appointment at your schedule time. If you are more
than 15 minutes late, we will consider it a failed appointment and you will have to reschedule. After missing your second appointment without
advanced notice, you may be charged a failed appointment fee.
Our office is committed to helping you maximize your insurance benefits and communicate effectively with your insurance provider. We do encourage
all of our patients to become familiar with their own coverage, as each plan varies. We are in-network providers with HealthPartners, Delta
Dental and United Concordia. If your provider is not on our in-network list, we encourage you to call them directly to see if you are able
